Becoming a Patient Proxy
At The Bay Medical Practice, we understand that some patients may need assistance in managing their healthcare, including booking appointments, requesting prescriptions, or accessing medical records. If you care for someone and need to act on their behalf, you may be able to apply for proxy access to their medical records.
Proxy access allows a nominated individual (such as a family member, carer, or legal representative) to access certain aspects of a patient’s healthcare information and services.
Who Can Apply for Proxy Access?
You may apply for proxy access if you are:
- A parent or guardian of a child under 11 years old.
- A carer, family member, or legal guardian for an adult patient who has given their consent.
- A registered carer acting in the best interests of a patient who lacks capacity.
- A person with Power of Attorney for Health and Welfare for a patient who is unable to manage their healthcare independently.
For children aged 11-15, proxy access is granted at the GP’s discretion and is typically limited to appointment booking and prescription requests. Full access may be restricted due to confidentiality rules.
What Can a Proxy Access?
Depending on the level of access granted, a proxy user may be able to:
- Book and cancel appointments on behalf of the patient
- Request repeat prescriptions
- View medical records, including test results and immunisations, if permitted
- Update patient contact details
Access levels can be tailored based on the patient’s preferences and GP approval.
How to Apply for Proxy Access

Collect a Proxy Access Form
- You can download the form from our website or collect one from reception.

Complete the Form
- The patient must sign the form to give consent, unless they lack capacity.
- You will need to provide proof of identity, such as a passport or driving licence.

Submit Your Application
- Bring the completed form and identification to the practice reception.
- A GP or practice staff will review the request before granting access.

Approval and Setup
- Once approved, you will receive login details for online access via Patient Access or the NHS App.
- If access is declined, we will provide an explanation and discuss alternative options.
Power of Attorney and Access for Patients Who Lack Capacity
If the patient is unable to consent due to a medical condition, a Lasting Power of Attorney (LPA) for Health and Welfare may be required. You will need to provide a legal document confirming your status as the patient’s legal representative.
If no LPA is in place, the GP will assess whether proxy access can be granted based on the patient’s best interests.
Security and Confidentiality
- Proxy access is granted at the patient’s discretion and can be revoked at any time.
- The practice follows strict confidentiality rules to ensure patient information remains secure.
- Misuse of proxy access may result in suspension of access rights.
