We welcome new patients who live within our practice boundary. A practice boundary map is available on this website to help you check if you are eligible to register.

You can register online or in person, and in urgent cases, we offer a priority registration process for those who need immediate care.


Register Online – The Fastest Option

The quickest way to register is by completing our online registration form via the NHS digital service:

Register Online with The Bay Medical Practice

  • The form takes approximately 15 minutes to complete.
  • You need to complete the registration in one session.
  • If registering more than one person, complete all forms before submitting them together.
  • If registering a child aged 15 or under, register yourself first before registering the child.

Once your details are received, your registration should be processed within five working days.

If you require medical help while waiting, visit NHS 111 online.


Registering In Person

If you prefer to register in person:

  • Visit the Sandown or Shanklin branch reception and collect a registration form.
  • Bring proof of identity (passport or driving licence) and proof of address (utility bill or council tax letter).
  • Complete a new patient health questionnaire to help us understand your medical history.

Our reception team will assist you with the process.


Urgent Registrations

If you require urgent medical care and are not yet registered with a GP, please contact us as soon as possible.

Who May Need an Urgent Registration?

  • Patients with urgent medical needs who are not yet registered.
  • Those moving to the area who require ongoing treatment or medication.
  • Patients needing palliative care or recently discharged from the hospital.
  • Pregnant patients requiring immediate antenatal care.

How to Request an Urgent Registration

1. Online Registration (Fastest Option)

  • Complete the online registration form and indicate any urgent medical needs.
  • Once submitted, contact the practice to confirm receipt.

2. Register in Person

  • Visit the Sandown or Shanklin reception and request an urgent registration.
  • Bring proof of ID and relevant medical history, if available.

3. Contact the Practice

Temporary Registration for Urgent Care

If you are staying in the area for up to three months and need medical care but do not want to permanently change GP surgeries, you can apply for a temporary registration.

  • Visit reception and complete a GMS3 Temporary Services Form.
  • You will be temporarily registered and able to access GP services during your stay.

Catchment Area (Practice Boundary)

Named GP

At The Bay Medical Practice, every patient is assigned a Named GP who is responsible for coordinating their overall healthcare and ensuring appropriate care and support is in place. This GP oversees your medical records, medication reviews, and general care management, ensuring continuity and coordinated care.

Having a Named GP does not limit your choice. You are welcome and encouraged to see any GP or healthcare professional within our team, and having a Named GP does not affect appointment availability or your choice.

How Do I Find Out My Named GP?

  • Log into your SystmOnline account, where your Named GP is clearly displayed in your medical record.
  • Contact the practice directly:

  • Visit Reception for help with your enquiry or to update your preferred GP.

If you haven’t yet signed up for online services, please ask reception about registering for SystmOnline.

Accessing someone else's information

Proxy access allows a nominated individual, such as a family member or carer, to manage a patient’s appointments, prescriptions, and medical records on their behalf. This can be particularly useful for children, elderly relatives, or those requiring support due to medical conditions.

To apply for proxy access, you will need to complete a proxy access form and provide identification. Access levels can be tailored based on the patient’s preferences and GP approval.
