Fit Notes (Sick Notes)
A fit note (MED3 Certificate) is a statement from a healthcare professional confirming whether you are fit for work or need time off due to illness. Fit notes provide evidence for employers or benefits claims if you are unwell for an extended period.
Do I Need a Fit Note?
If You Are Off Work for 7 Days or Less
You do not need a fit note from a GP. Instead, you should self-certify your illness.
- When you return to work, your employer may ask you to confirm your sickness absence.
- You may need to:
- Complete a self-certification form (SC2) from your employer or download it from the GOV.UK website.
- Provide written confirmation (email or letter) as agreed with your employer.
If You Are Off Work for More Than 7 Days
If you are unwell for more than 7 consecutive days, including weekends and bank holidays, you will need a fit note from a GP or another healthcare professional.

Backdating and Extending a Fit Note
- Backdated Fit Notes – A GP can issue a fit note for a past date if necessary, but they cannot issue one for future dates.
- Extending a Fit Note – If you need an extension, you can request one through our online form.
Returning to Work
- A GP can advise if you may be fit for work with adjustments, such as altered hours or lighter duties.
- Your employer should consider these recommendations, but they do not have to follow them.
- You do not need a fit note to return to work—you can go back when you feel well enough.

A fit note may be needed as medical evidence if you are applying for:
