If you have been asked by the practice to monitor your blood pressure (BP) at home, you can submit your 3-day or 7-day blood pressure readings quickly and easily online.
Regular BP monitoring helps us better understand your health and guide the best treatment for you.
🩺 How to Take Your Blood Pressure at Home

Sit quietly for 5 minutes before taking a reading.

Avoid caffeine, smoking or exercise for 30 minutes beforehand.

Take two readings, one minute apart, and record both.

Do this in the morning and evening at roughly the same times each day.

📋 How Long Should I Monitor My BP?

3-day readings: If your clinician has requested a 3-day monitoring period.

7-day readings: If your clinician has asked for a full week’s readings or if you are following home blood pressure monitoring guidelines.
Your clinician will advise which form you should complete.
📝 How to Record Your Readings
When recording your readings, please include:

Your full name and date of birth

The date of each reading

Morning and evening blood pressure readings (both readings if two taken)

Any comments about symptoms, if relevant

If your blood pressure readings are consistently very high (above 180/120) or you feel unwell (such as chest pain, breathlessness, or severe headache), seek urgent medical advice immediately.
